Physical Disability

Goals of Physical Disability Waiver:

Provides service funding and individualized supports to maintain eligible person in their own homes or communities who would otherwise require care in a medical institution.
Services are designed to be flexible to meet the needs of each consumer.

Services Available: Consumer-Directed Attendant Care (CDAC)

Eligibility:

Have a physical disability that is a severe, chronic condition, which is attributable to an impairment that results in substantial functional limitation in major life activities.
Be between the age of 18 and 64 years.
Be blind, or disabled, as determined by the receipt of Social Security Disability benefits, or by a disability determination made through the Division of Medical Services.
Consumers shall access all other services for which they are eligible and which are appropriate to meet their needs as a precondition of eligibility.
The consumer must be ineligible for the Mental Retardation Waiver.
The consumer shall be eligible for Medicaid.

These services are currently being provided in Page, Mills, Montgomery, Fremont, and Pottawattamie counties.
