Goals of
MR-Waiver
Program:
-
For people with
disabilities to become independent, integrated members of the community.
-
For services to be
individualized and flexible to meet outcome-based goals of the consumer.
-
To provide more
one-on-one training/support to assist each individual in becoming more
self-sufficient.
-
To provide settings in
which the individual’s rights are practiced along with learning
responsibility.
Services
Available:
-
Supported Community
Living:
Up to twenty-four hours of support each day to assist the individual with
their daily living needs in their own apartment, home, or shared living
arrangements.
-
Respite Care:
Short-term relief provided in the absence of the family, or caregiver,
such as vacation or a night out.
-
Community Employment:
Assistance in securing and maintaining a job in the community.
-
Home and Vehicle
Modifications:
NPI can provide assistance to help people make modifications, if needed,
to their home or vehicle to make it accessible.
-
Consumer-Directed
Attendant Care (CDAC):
Assist consumer with self-care tasks that the consumer would typically do
independently if the consumer were otherwise able.
Eligibility:
-
IQ of 75 or less,
with primary diagnosis of mental retardation.
-
Three or more
deficits on an assessment of 14 functions of daily living.
