Goal of the Elderly Waiver Program:

To provide services for senior citizens to help them continue living as independently as possible in their home or community.

Services Available:

• Respite Care

• Chore Service

• Home and Vehicle Modification

• Consumer-Directed Attendant Care (CDAC)

Eligibility:

• Persons 65 or older

• Receiving Medicaid Funding

• Requiring Intermediate Level of Care as assessed by Case Manager
