Goals
of Brain Injury Program:

To provide service
funding to support individuals in achieving their life goals.

To provide
individualized supports, which allows a person to stay in the home, or return to
their community from an institution.

Supported Community
Living

Respite Care

Community Employment
-
Prevocational
Services:
Services aimed at preparing a consumer for paid, or unpaid, employment.
These services are not job-task oriented.
It includes teaching the consumer job readiness skills.

Consumer-Directed
Attendant Care (CDAC)
Eligibility:
-
Diagnosis
of a brain injury
-
Eligible
for Medicaid
-
Assessed
by the Iowa Foundation for Medical Care (IFMC) to be able to live in a home, or
community-based setting, where all medically necessary service needs can be met
by the HCBS-BI program.
-
The
consumer must be aged one month to 64 years of age.
-
Must be
eligible for Case Management services each quarter
